Everytime I try and do a pattern it ends up completely different, please tell me if this happens to you and how to fix it.

I dont have any specific thing im talking about, I’m just talking about in general, I have never been able to complete a pattern or video or something like that without it screwing up.

Maybe a crochet hook size thing / tension?
I crochet with a very loose tension for example and always have to go down a hook size at least.
