Oof yarn barf I think that happens when people squeeze the yarn and it gets all tangled in the inside, if it gets too bad to detangle I usually cut it and work out the knot with a smaller section or if it’s too late for that just cut the knot section off and use the invisible join/knot/magic knot method to reattach lol

I can usually get it unknotted. But I am very patient.

Yeah I only had to do it a few times when I accidentally pulled the wrong piece of yarn and the knot became to tight to open up
