It might be because you have lost a stitch at the end. When you come to the end of the forward pass, and pull the loop through the last stitch of the previous row, make sure that there are 2 “columns” to the left of the loop you are pulling through. On the return pass, the first loop is done on its own, not connecting 2 loops. I hope you can see it in the photos?

Yes, when you incert the hook at the end, look at the edge if your project - there should be 2 “columns” of thread visible and then the body of the hook.
I want it to look like all the other Vs before it right? Under 2 loops but inserting in middle for knit stitch.
