Depends how you use it. If you say cc to red, it means color change. If you say starting in cc, it means contrasting color.

Either way you should or could specify it at the beginning of your pattern

Depends on how you are using it. If it is after a row or round I just write out change to color B. If it is mid row/round and it is all the same stitch (sc), I just put 32A, 14B, 2C. If it is mid row/round with different stitches, I put “in col A (2sc, 3hdc, dc), in col B (2dc, 3hdc),” etc.
You could even do CC(B), CC(D), CC(color)
Whatever you choose to do, it is best to put a note at the beginning of the pattern explaining your color changing method.
